Getting Started

Once you make contact with me and set an appointment for an Initial Evaluation, completing necessary paperwork is the next step. Reading and filling out some of these forms ahead of time will maximize the time we have to talk about the issues that have brought you to seek help.

If you want to complete preliminary paperwork prior to your first appointment, click on each of the forms below to print them out. If you have United Healthcare for your insurance provider, please complete the Initial Wellness Assessment below. Please all completed paperwork to your first appointment.

If you have questions or concerns about anything in the paperwork, we can either discuss these by phone or at your first visit.
